AIDS Case Management
Initial comprehensive assessment and periodic reassessments
Psychological assessment
Individual comprehensive service plan
Face-to-face client contact at least every 60 days
Coordination of client care
Client involvement in treatment planning

AIDS patients

Emergency Response Capacity/Responsibility:

Housing, food, gas, utility and rent for eligible clients
